Meet Our Care Managers
Diane Schwartz, MSW, LCSW
In the early years of Diane’s social work career, she acquired experience working with families and children with the Department of Social Services. She strived to help families remain intact by developing a trusting relationship with the parents and access needed resources to create a healthier family unit. As she continued to work with families and children, she changed directions and became a Certified Parent Educator with St. Louis Parents as Teachers program. She thoroughly enjoyed being a support to first time parents and watching their infant grow and develop.
As a Licensed Clinical Social Worker for over 10 years, Diane has experience assisting people with mental illness remain independent in the community. Sharing knowledge about mental illness and medication as well as environmental and psycho-social factors helps her understand and develop a plan for wellness with individuals. She has experience working with adults who have co-occurring issues (addiction and mental illness) and through Motivational Interviewing and mental stability, Diane has witnessed people attain sobriety and wellness.
As a Psychiatric Care Manager, she enjoys helping individuals on their path to wellness. Each person’s journey is different and unique; yet the ultimate goal is wellness and contentment with life. While learning to manage mental illness has its own unique barriers, they are not insurmountable. Diane’s goal is to help people overcome them by understanding the illness; develop coping skills, a support system, and a medication regimen addressing physical and psychiatric needs. It is a collaborative effort and she is here as your personal navigator.
